We’re currently receiving a number of registration forms that are incomplete or missing important information. To help us process your registration quickly and avoid delays, please make sure of the following:
All sections of the registration form and health questionnaire must be fully completed.
Unfortunately, we will need to reject any incomplete forms.
Temporary Registrations
If you’re registering as a temporary patient, please tell us:
The reason for the temporary registration (e.g. student, staying in a care home, visiting the area)
The length of time you will be staying — either up to 16 days or up to 3 months (90 days)
Registering a Child Under 16?
Please make sure to include the parent or legal guardian’s details on the form.
